It could be a issue if there is an inconsistency in a policy, i.e. 
"valid Policy Development Process".

Does Section 5.4.6.2 apply to the Section 5?

Section 6.5.4.1 refers to Section 4.4 of the document.  There isn't a 
Section 4.4 in the document.
