I sent a template to the RPD mailing list.  I suggest that you use it 
as a guide to format the proposal about "Transfer of IPv4 addresses 
to any entity".  You are listed as "IP Consultant" in this 
proposal.  Is that your affiliation?

Please request a unique identifier from AfriNIC and post your 
proposal to rpd at afrinic.net.
