I have 2 different v6 tunnels occasionally running at my house, but
this hardly counts in the grand scheme of things ;-)

yes, from this list, it's clear that there are a few in SA (TENET I
think and .za and .ke  ccTLDs).

Tunisia (Tunisian Internet Agency, the national gateway/sole
provider?) has had a v6 production network for ~4 or 5 years IIRC.

Besides Tunisia, no.

For many I speak to, it's about lack of compelling need to start deployment.
